To purchase, fill out the form above and return it in your child’s backpack with a check
made out to “Birch Meadow PTO“. Certificates $50 and under will go home in your child’s backpack,
over $50 will be in the school office to be picked up at your convenience. Fundraising certificates are not
redeemable for cash and may not be used for purchases less than the value of the fundraising certificate.
